Implementation of Collaborative Drug Therapy Management Services in a Large Public Health Care System in the United States

This session will review the implementation and outcomes of a Collaborative Drug Therapy Management (CDTM) program at a large public health care system in the United States. Clinical pharmacists are uniquely trained to address key issues surrounding management of diabetes, including the pharmacotherapy management, diabetes self-management education and medication adherence. CDTM allows pharmacists to co-manage patients under a collaborative practice agreement with a patient’s primary care physician.

Joshua P. Rickard, PharmD, MPH, BCACP

Josh Rickard is currently a Senior Director in the Office of Ambulatory Care and Population Health at New York City Health + Hospitals. In this position, Dr. Rickard leads the effort to build and maintain Collaborative Drug Therapy Management Pharmacy within the health system.

Dr. Rickard obtained a doctorate of pharmacy degree from the University of South Carolina and his Master of Public Health from the London School of Hygiene and Tropical Medicine. He completed his residency training at the James J. Peters VA Medical Center in the Bronx, NY and Intermountain Healthcare in Salt Lake City, UT specializing in Ambulatory Care Clinical Pharmacy. Dr. Rickard currently a Board-Certified Pharmacotherapy Specialist and Ambulatory Care Pharmacist, as well as a Certified Tobacco Treatment Specialist and Diabetes Care and Education Specialist.
