Person-Driven Outcomes: Driving Care That Matters

There is broad agreement that individuals’ priorities and health goals should guide their care, but existing quality measures do not effectively evaluate what is most important to individuals, particularly older adults with multiple chronic conditions, serious illness or frailty. Current health care quality measures use a “one-size-fits-all” approach that does not work when individuals face complex trade-offs in determining the right course of treatment or when they need services and supports beyond traditional medical care. The person-driven outcomes approach individualizes measurement based on what people identify as most important. Quality measures created on this approach have the potential to fill a critical gap in the current system of value-based payment. This course will discuss implementing person-driven outcomes in clinical practice and incorporating the patient voice into quality measurement.

Faculty

Caroline Blaum, MD, MS
Dr. Caroline S. Blaum joined NCQA from NYU Langone Health where she was the Diane and Arthur Belfer Professor of Geriatric Medicine and Director of the Division of Geriatrics and Palliative Care from 2012-2020. At NYU, Dr. Blaum maintained an active clinical practice and supervised clinical education for medical trainees in geriatrics and palliative care.  She led an extensive reach research program in translational research that was supported by the NIA, AHRQ, PCORI, and The John A Hartford Foundation concerning models of care for vulnerable populations, multiple chronic conditions, frailty, and diabetes in older adults. Prior to going to NYU, Dr. Blaum was Professor and Assistant Dean for Population Health at the University of Michigan Health System where she led UM’s experiences in Medicare managed care, ACO’s and PCMH. Dr. Blaum has been active in national policy work related to delivery system redesign and quality of care for older adults and people with multiple chronic conditions and has worked with the American Geriatrics Society, the AMA PCPI, and NQF.

Angelia Bowman, MS, MSPM
Angelia Bowman is a Director in the Research and Analysis Department at the NCQA. In this position, Ms. Bowman manages and directs the coordination and implementation of projects related to performance improvement for populations with multiple chronic conditions and/or serious illness. She has substantive knowledge in managing and directing projects with learning collaborative and quality improvement components at national, regional, and state levels. Prior to joining the NCQA, Ms. Bowman served as the project director for the John A. Hartford Foundation’s Change AGEnts Initiative at the Gerontological Society of America and served as a project and program manager for the Robert Wood Johnson Foundation’s Aligning Forces for Quality at the George Washington University’s Milken Institute School for Public Health.  

Scott Mancuso, MD
Scott Mancuso is Medical Operating Partner and Chief Medical Officer at Rubicon Founders, Honest Medical Group, and Evergreen Nephrology. Prior to this position, Dr. Mancuso was Chief Clinical Officer, West Region, at Landmark. Prior to joining Landmark, Dr. Mancuso held numerous clinical medical leadership positions at CareMore Medical Enterprises/Anthem, Inc. As Chief Medical Officer of CareMore Inside, Dr. Mancuso led the clinical assessment of and collaboration with health system and health plan partners in their transformation to value-based care models. Dr. Mancuso also has experience in hospital medicine, initially as a Hospitalist provider in Scottsdale, AZ, then as Associate Medical Director at Abrazo Healthcare, Vanguard Health Systems, in Phoenix. He also has experience providing palliative care consultation and hospice care. After completing his medical degree at Albany Medical College, Dr. Mancuso completed his residency in Internal Medicine at the University of Arizona at Banner University Medical Center. He is board certified in Internal Medicine and Hospice & Palliative Medicine. 

Michael Mason, MD
Michael D. Mason, MD, is Regional Co-Chair for Geriatrics and Continuing Care for Kaiser Permanente. In his tenure at Kaiser, he has developed systems to support geriatrics and supportive care and define and develop care of adults with serious illness. Previous to Kaiser, Dr. Mason practiced family and geriatric medicine and was Clinical Instructor of Family Medicine with the UCLA School of Medicine. He recently received grants to participate in NCQA Learning Collaborative studies to help devise and set standards for patients with serious illness. Dr. Mason received his MD at the USC School of Medicine and completed his residency at Kaiser Permanente Woodland Hills Family Medicine.

Aanand Naik, MD
Aanand Naik, MD, is Professor and a Robert J. Luchi, MD, Chair in Geriatric Medicine and the Chief of the Geriatrics and Palliative Medicine Section at Baylor College of Medicine. Dr. Naik also direct the Implementation Science and Innovations program at the Houston Center for Innovations in Quality, Effectiveness, and Safety (IQuESt), affiliated with the Michael E. DeBakey VA Medical Center (MEDVAMC). He has expertise in geriatrics and health outcomes research, with training in testing and implementation of shared decision making and health services interventions assisting older adults with multiple morbid conditions, including the Patient Priorities Care approach.
