While early success with the model shows feasibility, improved outcomes and lower costs for certain patients with select conditions and building consensus across providers, payers and policymakers is key for ensuring long-term success for this emerging model. Join NCQA and Hospital at Home experts for a 90-minute roundtable discussion of early successes and emerging policies and infrastructure considerations.
