CCE Quarterly: Building with a Blueprint: Taking a Patient-Centered Specialty Practice from Medical Neighborhood to High Performing Network

Providence Heart Clinics (PHC), the largest cardiovascular practice in Oregon, was the first cardiology practice in the country to be NCQA recognized as a Patient Centered Specialty Practice. Over the last 10 years, PHC has used PCSP standards as the blueprint to build strong ties with patient centered medical homes (PCMH) in the virtual medical neighborhood, standardize workflows, and guide practice improvement. With 20 PCSP practice sites in Oregon, PHC has been well prepared for pandemic challenges during COVID-19.

In 2022 PHC entered a new risk-bearing, value-based alternative payment contract with 63 organizations in Oregon, forming a High Performing Network (HPN). Using PCSP as a blueprint, PHC is leading specialty care transformation across the HPN to deliver high value cardiovascular care.

During this webinar, we will share our ongoing innovation projects with the NCQA community, including telehealth, e-consult, referral review, result reporting and virtual waitlist. These non-encounter-based care models, previously not supported by fee-for-service payment models, aim to reduce unnecessary encounters, accelerate appropriate care and enhance patient experience.

What You Will Learn

  •     Leverage NCQA’s standards as a blueprint for relationships and workflows, connecting specialists with primary care in the medical neighborhood.
  •     Design and implement digital care strategies to reduce unnecessary encounters, accelerate appropriate care and enhance the patient experience.
  •     Outline how to disrupt organizational silos to deliver care that trends toward establishing a physician-led, high-performing network.
