CCE Quarterly: Building with a Blueprint: Taking a Patient-Centered Specialty Practice from Medical Neighborhood to High Performing Network

In 2022 PHC entered a new risk-bearing, value-based alternative payment contract with 63 organizations in Oregon, forming a High Performing Network (HPN). Using PCSP as a blueprint, PHC is leading specialty care transformation across the HPN to deliver high value cardiovascular care. 

During this webinar, we will share our ongoing innovation projects with the NCQA community, including telehealth, e-consult, referral review, result reporting and virtual waitlist. These non-encounter-based care models, previously not supported by fee-for-service payment models, aim to reduce unnecessary encounters, accelerate appropriate care and enhance patient experience.

Xiaoyan Huang, MD, MHCM, FACC

Xiaoyan Huang, MD, MHCM, FACC is the Chief of Clinical Cardiology at Providence Heart Institute, Portland, Oregon, where she leads clinical operations and care transformation for advanced heart failure, electrophysiology, general cardiology and interventional cardiology subspecialties. She is the immediate-past President of Medical Staff at Providence Milwaukie Hospital.

Danielle Christensen, BS

Danielle Christensen, MBA candidate in 2023, is Clinical Program Manager at the Providence Heart Institute in Oregon. She has more than 7 years of experience in the health care industry, in both ambulatory and acute care settings, with particular expertise in project management, group facilitation, business development and strategy, planning, change management, and Epic electronic medical records. Ms. Christensen is a candidate for a master of business administration degree and has a Bachelor of Science degree in Organizational Communication from University of Portland.

Fatin Yousufzai, MSHP

Fatin Yousufzai is a Manager of Recognition Policy at NCQA. Her responsibilities include the creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to becoming a manager in Recognition Policy, Fatin was a Policy Analyst on the Performance Measurement team at NCQA, where she supported HEDIS measure development, maintained derivative HEDIS publications, and produced HEDIS Value Set Directories (VSDs).

This year she celebrates her 3rd year at NCQA. Fatin earned her B.S. in Health Sciences from Howard University and M.S. in Health Policy from George Mason University.
