A Call to Action

Cardiovascular disease is the leading cause of death in the United States. The deadliest type is atherosclerotic cardiovascular disease (ASCVD), characterized by a buildup of cholesterol plaque in the arteries. Although the rate of heart-disease related deaths declined for decades, progress has stalled, and we are now seeing an uptick. 

It’s crucial to educate health care providers and patients on the importance of cholesterol screening, a heart-healthy lifestyle and life-saving treatment options that can reduce the risk of heart attack and stroke. The Cholesterol Health Toolkit includes resources for patients, providers and care teams to help screen for and manage high cholesterol. 

The toolkit includes five resources:  

  • A patient handout and patient pamphlet on cholesterol and how to initiate conversations with providers. Available in English and Spanish. 
  • A provider handout and provider pocket guide on ASCVD risk factors and cholesterol screening guidelines. Available in English and Spanish. 
  • A population health management roadmap highlighting best practices for patient-centered care, and providing real-world examples of effective quality improvement activities.
