Healthcare organizations play a critical role in identifying and closing gaps in care. Health systems often see disparities firsthand, yet may lack clear, usable tools to quantify those gaps, understand patterns across populations and track whether interventions are making a difference over time.

The Playbook is designed to support healthcare leaders and their quality teams in using advanced health equity analytics to align and compare efforts within their institution, coordinate with partner organizations and drive targeted interventions to address identified gaps in care. Developed by NCQA and informed by real-world health systems and health plans, the playbook offers practical guidance on applying health equity scoring approaches using existing quality measures and data, so that insights can translate into action at the point of care.

What This Playbook Covers

The playbook provides guidance on how organizations can:

• Select and build a health equity scoring approach that aligns with organizational goals and data capacity.
• Analyze disparities across and between quality measurement focus areas and populations using composite health equity scores.
• Interpret and apply results to support quality improvement strategies and care delivery interventions.
• Track performance over time or across units to understand the impact of interventions.
• Support organizational alignment, garner leadership buy-in, and build accountability through consistent, transparent measurement.

The guidance is intended to be adaptable and applicable across different organizational contexts.
