DESCRIPTION:About this Webinar Training\nPCMH 201: Implementation Annual 
 Reporting 2027\nThis webinar will help PCMH practices 
 understand the larger goals of Annual Reporting and prepare 
 for annual submission under the Patient-Centered Medical 
 Home program. Faculty will highlight ways to demonstrate 
 sustained recognition status through maintenance and 
 continuous quality improvement efforts using data that a 
 practice is already collecting internally and for other 
 sources (e.g.\, state initiatives). Timelines for 
 deliverables and performance expectations will be key 
 discussion items.\n\nWhat You Will Learn\nAt the conclusion 
 of this program\, participants will be able to:\n\n\nOutline 
 the Annual Reporting requirements for PCMH 
 recognition.\nDescribe the Annual Reporting process\, 
 including timelines\, submission and audit.​\nInterpret 
 Annual Reporting requirements for reporting year 2027.\nHave 
 confidence navigating QPASS Measure Reporting tile\, after a 
 demonstration.\n\nWho Should Attend\nThis webinar is geared 
 towards individuals with one year of experience\, or more\; 
 collecting\, reporting\, and acting upon annual reporting 
 data\; or those who need a refresher. This program is 
 designed for individuals in the practice that work with 
 patients\, and with PCMH recognition.\n\nFaculty\nAli 
 Hulten-Stroop\, MSW\, MPA\, PCMH CCE\n\nAli Hulten-Stroop is 
 currently a manager of Recognition Policy at NCQA. Her 
 responsibilities include the creation and maintenance of 
 Recognition Program policies and resources. \n\nPrior to 
 joining NCQA\, Ali spent a decade as a clinical social worker 
 specializing in psychosocial support throughout the 
 continuum of cancer care\, end-of-life care\, and bereavement 
 support services. She has successfully helped develop and 
 maintain psychosocial support programs for cancer centers 
 for ASCO certification. \n\nFor the last several years she 
 has worked in the Medicare and Medicaid policy space\, with a 
 focus on quality improvement and implementing digital care 
 management tools for providers to enhance their care 
 delivery in multiple Value-Based Care programs.\n\nAli has a 
 strong interest in policy\, program management\, and quality 
 improvement in health care\, advocating to ensure everyone 
 has access to quality and affordable health care. She is 
 also passionate about building an interconnected digital 
 network to better measure care delivery outcomes and improve 
 self-managed care. She obtained her MSW from San Diego State 
 University\, and her MPA from UNC Chapel Hill. \n\n \n\nJess 
 Tomko\, PCMH CCE\n\nJess Tomko is a Manager of Recognition 
 Policy at NCQA. Her responsibilities include the creation 
 and maintenance of PCMH resources and providing support to 
 practices going through transformation or annual review. 
 \nPrior to NCQA\, Jess spent over 13 years in healthcare 
 administration\, where she successfully led primary care\, 
 specialty\, and surgical practices under the direction of  
 various stakeholders (private practice\, community health 
 systems\, private equity\, etc.). She effectively led practice 
 transformations\, strategic planning\, M&A’s\, compliance 
 integrations\, and quality initiatives throughout her roles. 
 Jess also has extensive experience with various EHR's\, HIT 
 and data reporting. Jess’s professional interests include 
 healthcare operations\, compliance\, and quality improvement. 
 She has a particular interest in health equity and practice 
 burnout.\nJess earned her B.S. from George Mason University 
 and has been an NCQA Patient-Centered Medical Home Certified 
 Content Expert (PCMH CCE) since 2023.\n\n \n\nErika 
 Mountcastle\n\n\nErika Mountcastle is a Recognition 
 Operations Representative at the National Committee for 
 Quality Assurance. Her primary responsibilities include 
 guiding practice sites through the recognition process\, 
 assessing readiness\, helping practices plan their approach 
 to recognition and providing technical assistance. She has 
 been serving NCQA accredited practices for 5 years. In her 
 role\, Erika brings together precision\, empathy\, and a 
 service‑driven mindset to support high‑volume\, 
 high‑impact recognition initiatives. She is known for her 
 positive spirit\, collaborative energy\, and interest in 
 continuous learning. Erika earned her B.A. from James 
 Madison University.\n\nContinuing Education\nThis live 
 course grants 2.0 required Continuing Education Unit (CEU) 
 points for PCMH Certified Content Experts.\n\nThis is a non
