DESCRIPTION:About this Webinar Training\nPCMH Recognition Annual 
 Reporting 2025: Continued Success\nThis webinar will help 
 PCMH practices understand the larger goals of Annual 
 Reporting and prepare for annual submission under the 
 Patient-Centered Medical Home program.\n\nFaculty will 
 highlight ways to demonstrate sustained recognition status 
 through maintenance and continuous quality improvement 
 efforts using data that a practice is already collecting 
 internally and for other sources (e.g.\, state initiatives). 
 Timelines for deliverables and performance expectations will 
 be key discussion items.\n\nWhat You Will Learn\nAt the 
 conclusion of this program\, participants will be able 
 to:\n\n\nOutline the Annual Reporting requirements for 
 PCMH recognition.​\nDescribe the Annual 
 Reporting process\, including timelines\, submission and 
 audit.​\nInterpret Annual Reporting requirements for 
 reporting year 2025.\nHave confidence navigating QPASS 
 Measure Reporting tile\, after a demonstration.\n\nWho Should 
 Attend\nThis webinar is geared towards individuals with one 
 year of experience\, or more\; collecting\, reporting\, and 
 acting upon annual reporting data\; or those who need a 
 refresher. This program is designed for individuals in the 
 practice that work with patients\, and with PCMH 
 recognition.\n\nFaculty\n\nJennifer Anglin\, MS\, CHES\, DHSc\, 
 PCMH CCE\nJennifer Anglin is Senior Manager of Recognition 
 Policy at NCQA. Her responsibilities include creation and 
 maintenance of PCMH and other Recognition resources and 
 providing support to practices going through transformation 
 and annual review. Prior to NCQA\, Jennifer worked for Purdue 
 University by assisting practices throughout the state of 
 Indiana on quality improvement and research projects as well 
 as supporting them to meet Medicaid's Promoting 
 Interoperability program and Medicare's MIPS program. 
 Jennifer's professional interests include health policy\, 
 research\, and education. She has a particular interest in 
 social determinants of health and the behavioral health 
 aspect of Patient-Centered Medical Homes. Her personal 
 interests include being with family and their pets and going 
 for walks with her husband and two German 
 Shepherds.\n\n Jennifer holds a BA in secondary education 
 from Valparaiso University\, a master's degree in health 
 science from Indiana State University and a doctorate in 
 health science from Indiana State University. Jennifer and 
 has been an NCQA Patient-Centered Medical Home Certified 
 Content Expert (PCMH CCE) since 2013.\n\n \n\n\n\nNoah 
 Hammes\, MPH\n\n\nNoah is a manager of recognition policy 
 within NCQA's Product and Customer Operations department\, 
 where he acts as a subject matter expert on behavioral 
 health recognition programs as well as a certified content 
 expert for NCQA's Patient-Centered Medical Home recognition 
 program. Noah Hammes has several years of experience in the 
 behavioral health and healthcare federal and state policy 
 spaces\, as well as a background in behavioral health and 
 public health program development and evaluation. Noah 
 received his BA from the University of Rochester and his MPH 
 from UNC Chapel Hill.\n\n \n\n\n\nMonica McGill\, Analyst\, 
 PCMH CCE\n\n\nMonica McGill is an Analyst in Recognitions 
 Operations at the National Committee for Quality Assurance. 
 As a PCMH CCE\, Ms. McGill guides practice sites through the 
 recognition process by reviewing their specific challenges 
 and assessing where the practice stands compared to the 
 requirements for Recognition. Once enrolled\, Ms. McGill 
 interacts regularly with practices through the online portal 
 to provide feedback on content related questions\, uploading 
 evidence\, payment issues\, account access\, as well as to 
 monitor progress.\n\nMs. McGill is a highly engaged\, 
 intuitive\, and collaborative leader who recently served as 
 NCQA’s DEI council co-chair where she worked to drive key 
 DEI initiatives and actions across the organization 
 beginning with the development of the DEI council charter. 
 Using DEI strategy and thought leadership\, Ms. McGill 
 coordinated efforts including NCQA programs around DEI 
 awareness and inclusion and overseeing DEI council 
 subcommittees.\n\nMs. McGill attended Virginia State 
 University and in addition to her PCMH CCE\, has earned 
 certifications in Health Care and the Aging Adult from 
 University of Maryland University College and Diversity 
 Equity and Inclusion in the Workplace from University of 
 South Florida\, MUMA College of Business. \n\nContinuing 
 Education\nThis live course grants 2.0 required Continuing 
 Education Unit (CEU) points for PCMH Certified Content
