SUMMARY:Introduction to PCMH 2017 New York\, NY
DESCRIPTION:Introduction to PCMH Program: Foundational Concepts of the
 Medical Home\nDecember 10-11|New York\, NY\n\nEarly Bird
 Registration Ends: Ocotober 26\, 2018\n\nDuration: 1.5 days
 \n\nCredits: 7.75\n\nAbout the Seminar\n\n\n\n\nThis program
is an in-depth guide to NCQA Recognition requirements and
the process for achieving PCMH recognition. Recognized
practices and clinicians have proved the relevance of the
NCQA PCMH standards to the Triple Aim. The 2017 standards
emphasize a practice wide commitment to sustaining the PCMH
transformation\, demonstrating patient care and population
health management\, care coordination and ongoing quality
 improvement.\n\nPractices are rewarded for integrating
behavioral healthcare and addressing the impact of social
determinants on patient needs. NCQA requirements are
positioned to qualify for the Merit-Based Incentive
Performance System (MIPS) Clinical Practice Improvement
Activities (CPIA) and to support other MIPS requirements
under MACRA.\n\nAdditionally\, a special discussion will be
held for New York State attendees and others interested in
learning about the New York State Patient-Centered Medical
Home (NYS PCMH) Recognition Program. Participants will be
able to hear first-hand and ask questions about this
exclusive model developed based on the NCQA PCMH model\,
which supports the state’s initiative to improve primary
care and promote the Triple Aim: better health\, lower costs
and better patient experience.\n\nJoin us for this thorough
review of the PCMH 2017 requirements and the documentation
needed to support an application for NCQA
Recognition.\n\nWhat You Will Learn\nAt the conclusion of
the program\, participants will have the knowledge they need
to:\n\n• Identify the core attributes of patient-centered
medical homes.\n\n• Identify required evidence and
determine how to present it.\n\n• Describe processes and
procedures that demonstrate transformation into the medical
home model.\n\n• Understand the roles of the NCQA
Representative and Evaluator and CCE. \n\n• Examine the
challenging requirements in a variety of practice
environments. \n\n• Outline the features and functions
of the Q-PASS system.\n\nWho Should Attend\nThis program is
designed for any health care professional\, health care
decision maker or consultant interested in getting the
inside perspective directly from expert faculty from NCQA in
evaluating a primary care practice and their ability to
become Recognized as a Patient-Centered Medical Home by
 NCQA. \n\nDate\, Seminar Hours\, Location and Fees\nDecember
