 D.O.\, MBA\, is a board-certified emergency room physician 
 with clinical\, entrepreneurial\, administrative and 
 operational experience. Dr. Agarwal’s mission is to help 
 with transition from a fee-for-service model of care to a 
 value-based model through patient and physician engagement. 
 Dr. Agarwal was Chief Medical Officer for a health analytics 
 platform\, health care consultant for Accenture\, Medical 
 Director in the Executive Physician Immersion Program at 
 Humana and is a VP in the Home Care Solutions Division at 
 Humana. He completed his Six Sigma Green Belt training.\nDr. 
 Agarwal received his MBA from the University of Illinois\, 
 his medical degree from Des Moines University and his BS in 
 finance and accounting from Washington University in St. 
 Louis. He completed his residency in Emergency Medicine at 
 the University of Florida and is Assistant Professor of 
 Emergency Medicine at the University of 
 Kentucky.\n\nCaroline Blaum\, MD\, MS\nDr. Caroline S. Blaum 
 joined NCQA from NYU Langone Health\, where she was the Diane 
 and Arthur Belfer Professor of Geriatric Medicine and 
 Director of the Division of Geriatrics and Palliative Care. 
 At NYU\, Dr. Blaum maintained an active clinical practice and 
 supervised clinical education for medical trainees in 
 geriatrics and palliative care. She led a program in 
 translational research that was supported by the NIA\, AHRQ\, 
 PCORI and The John A Hartford Foundation concerning models 
 of care for vulnerable populations\, multiple chronic 
 conditions\, frailty and diabetes in older adults. Prior to 
 NYU\, Dr. Blaum was Professor and Assistant Dean for 
 Population Health at the University of Michigan Health 
 System. Dr. Blaum has been active in national policy work 
 related to delivery system redesign and quality of care for 
 older adults and people with multiple chronic conditions and 
 has worked with the American Geriatrics Society\, the AMA 
 PCPI and NQF.\n\nJohn D. Halamka\, MD\, MS\nJohn D. Halamka\, 
 M.D.\, M.S.\, president of the Mayo Clinic Platform\, leads a 
 portfolio of platform businesses focused on transforming 
 health care by leveraging artificial intelligence\, connected 
 health care devices and a network of trusted partners. Prior 
 to his appointment at Mayo Clinic\, Dr. Halamka was Chief 
 Information Officer at Beth Israel Deaconess Medical Center. 
 As the International Healthcare Innovation Professor at 
 Harvard Medical School\, Dr. Halamka helped the George W. 
 Bush administration\, the Obama administration and 
 governments around the world plan their health care 
 information strategies. Dr. Halamka completed his 
 undergraduate studies at Stanford University\, earned his 
 medical degree at the University of California\, San 
 Francisco\, and pursued graduate work in bioengineering at 
 the University of California\, Berkeley. He completed his 
 residency at Harbor—UCLA Medical Center in the Department 
 of Emergency Medicine. He was elected to the National 
 Academy of Medicine in 2020.\n\nBruce Leff\, M.D.\nDr. Bruce 
 Leff is Professor of Medicine at the Johns Hopkins 
 University School of Medicine\, where he directs the Director 
 of the Center for Transformative Geriatric Research. He 
 holds joint appointments in the Department of Health Policy 
 and Management at the Johns Hopkins Bloomberg School of 
 Public Health and the Department of Community and Public 
 Health at the Johns Hopkins School of Nursing. He is a 
 national and international expert in Hospital at Home\, and 
 led the development of the first models in the U.S.\, 
 national demonstration studies Medicare Advantage and a CMMI 
 demonstration. He co-directs the National Home-Based Primary 
 Care Learning Network (https://improvehousecalls.org/)\, 
 whose mission is to advance the field of home-based medical 
 care through a coordinated program of quality improvement 
 and applied research. Dr. Leff has served on multiple NQF\, 
 NCQA and CMS technical expert panels. He is past-director of 
 the JHOME (home-based primary care) Program at Johns Hopkins 
 and is current Chair of the Geriatric Medicine Board of the 
 American Board of Internal Medicine of the ABIM Council. As 
 past -President of the American Academy of Home Care 
 Physicians\, he helped develop and implement the Independence 
 at Home Demonstration (section 3024 of the Affordable Care 
 Act).\n\nRaphael Rakowski\nRaphael Rakowski is Executive 
 Chairman\, Chief Development Officer and Co-Founder of 
 Medically Home\, pioneering the world’s first virtual 
 hospital. He is an American entrepreneur\, investor and 
 health care industry veteran. He is a public speaker and 
 advocate for changes to the American health care system and 
 is the subject of a documentary ("Mr. Rakowski") examining 
 the effects of the Holocaust on his childhood. Mr. Rakowski 
 was the CEO of American Healthways and co-founded 
 Intersection Partners\, which launched a series of health 
 care services businesses\, including Clinically Home\, the 
 predecessor model to Medically Home Group. He is a graduate 
 of Lehman College.\nContinuing Education\n\nIn support of

 for Continuing Medical Education (ACCME)\, the Accreditation 
 Council for Pharmacy Education (ACPE)\, the American Nurses
