 Barnes\, PCMH CCE\nThomasene is the Director of the 
 Patient-Centered Medical Home (PCMH) Program within the 
 Office of Ambulatory Care at New York City Health and 
 Hospitals (NYC H+H)\, the largest public healthcare system in 
 the nation.  She has more than ten years of healthcare 
 experience at NYC H+H\, spanning ambulatory care operations 
 and healthcare services transformation.  Over the course of 
 her career at NYC H+H she has collaborated with 
 interdisciplinary teams to align recognition efforts\, 
 developed patient-centered policies and procedures\, managed 
 various ambulatory care practices\, and has also assisted 
 with coordinating various initiatives and projects such as 
 the NYSDOH Hospital-Medical Home Demonstration Project\, part 
 of the NYS Medicaid Redesign program.  Thomasene holds a 
 Bachelor’s degree in Health & Society with a minor in 
 Psychology from the University of Rochester and earned PCMH 
 Content Expert Certification in 2016.  She has previously 
 supported NYC H+H practices under all versions of the PCMH 
 standards at the practice level and system wide at the 
 corporate office level.  Currently Thomasene is responsible 
 for coordinating the efforts of over 32 practice sites 
 across the system for the NYS PCMH Recognition program under 
 the 2017 standards.  Thomasene’s dedication to providing 
 health care to those in underserved populations continues to 
 propel her work expanding practice transformation efforts 
 that are patient-centered. Thomasene is a 2019 – 2020 PCMH 
 CCE Quality Award Winner.\nAmy Tennant\, PCMH CCE\nAmy 
 Tennant is a Policy Manager in NCQA’s Recognition Programs 
 Policy and Resources. In this role\, she creates and 
 maintains PCMH resources\, manages product updates and 
 provides policy support and expertise to customers\, PCMH 
 CCEs and NCQA Evaluators.\nShe started at NCQA in the 
 Product Development department where she supported the 
 development of recognition programs including the redesign 
 of the PCMH standards for 2017\, Patient-Centered Specialty 
 Practice School-Based Medical Home and the creation of the 
 Oncology Medical Home\, the first recognition program created 
 for a specific medical specialty.\nBefore starting her 
 career at NCQA\, she was an intern at MedStar Georgetown 
 University Hospital where she worked on reducing medication 
 reconciliation errors and incidence of outpatient falls. She 
 obtained her Six Sigma Black Belt and holds a BS in 
 Healthcare Management and Policy from Georgetown University 
 in Washington\, DC.\nContinuing Education\n\nIn support of 
 improving patient care\, the National Committee for Quality 
 Assurance is jointly accredited for Continuing Medical 
 Education (ACCME)\, the  American Nurses Credentialing 
 Center (ANCC)\, the American Academy of Physician Assistants 
 (AAPA)\, the American Psychological Association (APA)\, and 
 the Association of Social Work Boards (ASWB) to 
 provide Interprofessional Continuing Education for the 
 healthcare team. \nThis activity was planned by and for the 
 healthcare team\, and learners will receive 1.0 
 Interprofessional Continuing Education (IPCE) credit for 
 learning and change. \nThis educational activity is 
 approved for: 1.0 AMA PRA Category 1 CreditTM\, ANA CNE\,
