Disease Management Accreditation

DiseaseManagementAcc_full_RGB_lowres.jpg NCQA offers a flexible evaluation program, including Accreditation for organizations that offer comprehensive DM programs with services to patients, practitioners or both; and Certification for organizations that provide specific DM functions. These standards build on NCQA's years of Accreditation and Certification experience, detailed market research and input from industry experts and other stakeholders.

NCQA Accreditation or Certification will allow organizations that offer quality DM programs and services to receive the market advantage, high standards of quality and recognition from key contractors the industry demands. The NCQA Accreditation or Certification seal of approval may help your organization relieve delegation oversight from health plans and meet many request-for-proposal (RFP) requirements.

In-Depth DM Accreditation and Certification Information

Disease Management Performance Measures
NCQA has incorporated standardized performance measures that address dimensions of care for people with asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart failure and ischemic vascular disease (IVD) in the Standards and Guidelines for the Accreditation and Certification of Disease Management. The DM performance measures also include measures for tobacco use, influenza vaccination and pneumococcal vaccination, which apply to people in each domain.

NCQA is the first DM accreditation organization to utilize performance measures to assess the impact of programs on care for people with the above diseases.
