This annual report details performance trends, tracks variations in patterns of care and provides recommendations for future quality improvement. Now in its 15th year, the report provides an exclusive look at the performance data submitted by health plans around the country. Findings include:

This collaboration with Pfizer Inc. provides practical examples of how Patient-Centered Medical Home can help improve health care quality and value. It includes case studies of successful medical home initiatives and guides practices hoping to become patient-centered medical homes. The case studies showcase high-performing practices that are addressing contemporary challenges in primary care, and that have also earned NCQA’s highest medical home designation, PCMH Level 3 Recognition.

NCQA requires an honorarium and reimbursement of speakers’ travel expenses. The amount of honorarium is based on the speaker’s position in the organization, length of presentation, preparation time and travel time required. See a list of NCQA speakers.
