Important Note for 2008 Health Plan Accreditation ~ please read before purchasing your 2008 Survey Tool(s): 

If your organization is planning to bring a PPO product through 2008 Health Plan Accreditation at the same time as an HMO or POS product, depending on how your organization is structured you may need to purchase two Survey Tools. Please do not purchase a second Survey Tool at this time.  Purchase only one Survey Tool with as many licenses as you need. During the application process, NCQA staff will work with you to determine if you need to purchase a second Survey Tool. For more information, please review the application materials available for order below.  If you have additional questions, please submit them to the Applications and Scheduling Clarification System (ASCS).

Survey Tool for the Accreditation of Health Plans

The Survey Tool includes all information needed to prepare and submit materials for an NCQA survey. Organizations applying for accreditation must complete the Survey Tool and submit it to NCQA to begin the survey process.
