Survey Tool for Certification in Utilization Management/Credentialing
The Survey Tool includes all information needed to prepare and submit materials for an NCQA survey. Organizations applying for accreditation/certification must complete the Survey Tool and submit to NCQA to begin the survey process.  

Note: Use of the Survey Tool is required for organizations preparing for an accreditation survey.

For web-based products, NCQA does not make any commitments with respect to the use or performance of the Software and Services with browsers other than Netscape 6.0 or higher, or MS Internet Explorer 5.5 or higher. Please review the license agreement for web-based tools prior to purchase.
