The Physician Practice Connections^® (PPC) Program recognizes practices that have superior systems in place for connecting patients and doctors with information and with each other. An electronic medical record (EMR) is not required to achieve Recognition.  

Survey Tool for Physician Practice Connections
The Survey Tool is an invaluable resource containing all the information you need to apply for recognition. It incorporates the Standards and Guidelines and facilitates collecting information about how the practice meets the standards. (This is the version to use to apply for recognition). If you are applying as part of a sponsored program such as the Bridges to Excellence Physician Office Link, you are entitled to a discount.
