Medicare Advantage Modules
The MA Modules are essential resources for MCOs and PPOs that provide care under the MA program. These publications supplement the NCQA Accreditation survey with Medicare-specific requirements for the complete MA survey. The MA Module contains the Standards and Guidelines, data collection tools, an appendix of useful information and details about eligibility requirements, application procedures and policies and procedures.

Application for Accreditation for Health Plans

This application provides all materials required to apply for an NCQA Accreditation survey and includes pricing information, policies and procedures and the survey agreement. NCQA also offers a Corporate Survey option to national or regional organizations that provide centralized functions for local plans or local operating units.
