The 2012 HP MA Module is effective July 1, 2012. The 2012 HP MA Module is included in the 2012 HP Standards and Guidelines and the 2012 HP Survey Tool on the Interactive Survey System (ISS).

These publications supplement the NCQA Accreditation survey with Medicare-specific requirements for the complete MA survey. The MA Module contains the Standards and Guidelines, data collection tools, an appendix of useful information and details about eligibility requirements, application procedures and policies and procedures.

Note: The MA Module is included in the Web-based HP Standards and Guidelines and the HP Survey Tool on the Interactive Survey System (ISS).
