About the Program
This seminar is back by popular demand!
Join us January 25th and 26th as expert faculty from NCQA guide participants in this interactive workshop. NCQA’s President, Margaret E. O’Kane, will conduct the welcome and opening remarks for this very special program.
The faculty will examine how practices can demonstrate that they meet NCQA’s PPC®-PCMH™ requirements and discuss sample submissions for PPC-PCMH Recognition. Participants will learn how to identify documentation that does (and does not) meet the requirements and we’ll practice scoring each element. The group will identify strategies for enhancing and improving valid content and clarity of the application towards a Recognized Patient-Centered Medical Home and identify the aspects of the survey and evaluation process that may challenge or facilitate an efficient application and survey submission in a variety of practice environments.
This program is designed for any health care professional or consultant interested in getting the inside perspective directly from expert faculty from NCQA in evaluating a primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA.
The purpose of this educational program is to help participants:
- Outline the essential characteristics of a Recognized PCMH
- Identify the measurement and documentation criteria for each of NCQA’s requirements
- Facilitate the NCQA survey and evaluation process.

Continental breakfast will be served on both days. Lunch will be served on Day 1.
