Add-On Process

Step 1: Request an Add-On Survey From NCQA (two options) 

  • Did your practice previously submit an application through its online application account? 
  • Log in to the online application account. Go to the Submit Application page and click Request Add-on.
  • Your request will be processed in 1–5 business days and you will be contacted by an NCQA representative. 

Step 2: NCQA Creates Your Add-On Survey Tool 

  •  You do not need to buy a new Survey Tool; NCQA creates one from your previous survey. NCQA locks all elements that earned 100%; they are in read-only format. 
  • No further data entry or response is required for those elements. 
  • Documents from the previous survey are not merged with the new Survey Tool and are not accessible in the new Survey Tool. 
  • NCQA merges responses from the previous Survey Tool into the new Survey Tool and notifies you when the Add-On Survey Tool is ready for you to complete. 
  • Your practice may change responses to an element not scored at 100% and may attach new documentation. 
  • Your practice can keep its previous element score by not changing responses. If the previous score is retained, it is not necessary to reattach the previous documentation. 
  • LEAVE PREVIOUS COMMENTS AND DATE NEW COMMENTS. 

Step 3: Complete Your Add-On Survey Application 

  •  Complete and submit an application for the Add-On Survey through your online application account. 
  •  If you previously submitted a paper application, enter the license number when you “Add Information” to the practice site. 
  •  If you previously submitted through your online application account, your license number is already in the Add-On Survey record in your account. 
  •  All applicants complete “Add Information,” update and tie clinicians to the record and submit. You must click Submit on the Submit Application page. 

Step 4: Submit the Add-On Survey Application Fee · 

 For each site going through an Add-On Survey, use My EzPay in the online application to make a payment. 

Step 5: Complete and Submit the Add-On Survey 

  • NCQA sends an e-mail notification when the Add-On Survey application has been accepted. 
  • Upload new documents and change responses in the Add-On Survey Tool. 
  • Indicate the Add-On elements that need review in the Organization Background "Add-on Elements" tab. 
  • Submit the Add-On Survey. 

Step 6: NCQA Reviews Add-On Survey Tool

  • NCQA reviews the new Survey Tool and sends a notification e-mail with the results. You receive the recognition decision in 30–60 days. 
  • NCQA updates your practice’s survey to reflect changes in scoring or recognition level, if applicable. 
  • If a higher recognition level is achieved, your practice is issued a new certificate and the NCQA Web site and data feeds are updated.

Add-On FAQ

Q: If our practice submits for PCMH 2014 recognition, but receives a denial after the submission deadline (September 30, 2017), is the Add-On Survey an option?

A: No. Add-On Surveys are not an option after September 30, 2017. After that point, practices must come through the new recognition process under the PCMH 2017 standards (scheduled for release in March 2017). NCQA recommends that practices consider the deadline when planning their submission.

Q: Our practice has PCMH 2014 recognition. If we miss the submission deadline of September 30, 2017, is the Add-On Survey an option?
