Your practice is welcome to download personalize and distribute the release to your local media. Please send the release to firstname.lastname@example.org for review and approval before you send it out.

NCQA has created PCMH seals and graphics to help you champion your achievements. Follow the Guidelines for Advertising and Marketing Recognition Programs in all NCQA-related marketing activities.
Was your final recognition level lower than you would have liked? You may ask NCQA to reconsider its PCMH decision. You might also upgrade your recognition level—say, from Level 1 to Level 3—by coming through for an Add-On Survey. Did you achieve recognition status under PCMH 2011 and want to transfer your current recognition to PCMH 2014? You can apply for a Conversion if you meet the eligibility requirements..
Don’t let your recognition lapse! Recognition lasts 3 years, but you should start the renewal process at least 6 months before expiration. Check requirements for renewals if you are a recognized Level 2 or Level 3 practice and are interested in streamlined renewal. Multi-sites, see your requirements for PCMH 2014 renewal.
What happens if an NCQA-Recognized practice merges with another practice? What happens if a recognized practice is acquired? What happens if a clinician joins or leaves a practice? NCQA understands these are realities in modern health care. See our FAQs regarding mergers and adding/deleting clinicians.
Continuous quality improvement will not only maintain your PCMH 2011 recognition, but will also help sustain it.
