The provision of care requires a diverse team of professionals. Therefore, the team that is working to improve and transform our systems of care must also be diverse.
• Improvement team membership should match the clinic team make up; not only the clinical personnel. Every staff personnel play a vital role in the care process. Those who overlook this puts at risk the ability to institute changes that will produce outcomes they are looking for and are sustainable.
• Patients and families that are served, as well as the medical home neighborhood that works with us to care for patients, should not be neglected. Patients and families provide a lens on the important work in the medical home neighborhood that may not be seen.
