NCQA Practice Recognition Programs empowers employers, health plans, patients and consumers to make informed heath care decisions based on quality. Participation in a NCQA Practice Recognition Program demonstrates that you value quality health care delivery and the latest clinical protocols to ensure your patients receive the best care at the right time.

NCQA Recognition Programs for Practices

Patient-Centered Medical Home (PCMH) 
Recognizes clinician practices functioning as medical homes by using systematic, patient-centered and coordinated care management processes

Patient-Centered Specialty Practice Recognition (PCSP)
This program recognizes specialty practices that have successfully coordinated care with their primary care colleagues and each other, and that meet the goals of providing timely access to care and continuous quality improvement.

Patient-Centered Connected Care™ Recognition: Recognizing Trusted Partners for Patient Care 
This program evaluates how ambulatory care providers – such as urgent care centers, onsite employee health clinics and school- and retail- based clinics -- communicate and connect with primary care providers as they deliver care to shared patients.

Government Recognition Initiative & Projects (GRIP)
Works with government agencies to promote concepts of the Patient-Centered Medical Home and to recognize Federally-Qualified Health Centers (FQHCs) as medical homes.

Physician Practice Connections (PPC) (RETIRED)
Recognizes clinician practices that use up-to-date information and systems to enhance patient care.
