About the Program

The NCQA’s Patient-Centered Medical Home Recognition (PCMH) program plays an integral role in practice transformation and the improvement of quality health care. This advanced seminar further examines the criteria of the PCMH 2014 Standards. With the newly updated standards, released in the spring of 2014, we are setting the bar higher for practices to strive for excellence. We will take a deeper dive into the standards by helping to identify the challenges one may face based on historical data and advisory panel feedback. In addition to the standards, we will review acceptable documentation for the elements, examine scoring rules and discuss how a practice can create and maintain a continuous quality improvement culture. This course will discuss the integration of behavioral health, the team based approach to care management, and also identify the meaningful use (MU) requirements embedded in the standards. Our faculty will indicate how practices can receive credit for their MU achievements when seeking PCMH 2014 Recognition.

The advanced program is intended for individuals who have substantial knowledge of the NCQA PCMH requirements and have applied these requirements within a practice setting. Completion of this seminar is an eligibility requirement for NCQA’s Patient-Centered Medical Home (PCMH) Content Expert Certification Program.*

*Testing window for the 2014 version of the exam opens September 2014. PLEASE NOTE: This course does not satisfy the eligibility requirement for the 2011 version of the exam.

At the conclusion of this educational program, the participant will be able to:

  • Identify and address the challenging NCQA Patient-Centered Medical Home (PCMH) standards.
  • Describe all phases of the PCMH submission and review process, and the options and actions practices may choose to take after they have received their Recognition decisions.
  • Interpret the meaningful use criteria and EHR prevalidation to obtain credit in the aligned elements.
  • Identify opportunities for alignment with other NCQA recognition programs.
  • Establish and maintain a continuous quality improvement culture within a PCMH practice.

This interactive program is geared towards individuals with significant knowledge of the NCQA PCMH Standards and experience implementing those standards in a practice setting and who have attended the Facilitating Patient-Centered Medical Home 2014 Recognition seminar; and those who intend to obtain the individual Patient-Centered Medical Home (PCMH) Content Expert Certification credential.

NCQA designates this live educational activity for a maximum of 4.50 AMA PRA Category 1 CreditsTM.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
