About the Program

Transforming your practice into a medical home can be an overwhelming process. Learn some key steps to champion the effort in getting your team onboard. Get first-hand insight on lessons learned in redesigning care management coordination. You will have an opportunity to ask your questions to faculty.

  1. Define organizational values and create a culture open to change. 
  2. Describe how the roles of the care team members change in the a patient-centered medical home. 
  3. Recognize how to translate words into actions: What care management really means. 
  4. Develop a strategy to redesign the team to achieve optimum care coordination.

  NCQA provides electronic copies of seminar materials (slides, resources, etc.) to attendees before an event.

One week before a seminar or workshop, registered attendees will receive an e-mail with a link and instructions for accessing electronic presentation slides, handouts and reference materials. Materials can be printed in advance or downloaded onto a laptop, tablet or smartphone to view during the seminar or workshop.
